ML-12 Indoor Track Roster Reporting Form.
Use this form to send your entrants for the Championship Meet
Each team will be allowed to enter three athletes per event with no qualification standard. Additional event entries per team provided entrant meet the Division (D4) M.I.A.A. qualification standards. Teams are limited to two long jumpers and one team per relay.

 Individual entrant limit applicable 1 run + 1 field + 1 relay

PLEASE FOLLOW sequence in Roster text box:

bib#, First Name, Last Name, YOG, Event#1+Mark, Event#2+Mark, Event#3+RELAY

Time listed to tenth will be HT and to thousandth ELECTRONIC.  I will convert all times HT to ELECTRONIC using 0.24 differential. 

DO NOT LIST RELAYS ENTRANTS SEPARATELY, include as specified with bib# etc.
